Position Summary

Posting dates: 9/29/09 through 10/05/09 or until filled

Hours: 40 hours per week; 44 weeks per year over 12 months


Job Summary:

Prefferd bi-lingual (English - Hmong); cultural connections and familiarity with Hmong Community.

To assist in the identification and facilitation of early intervention services to young children with special needs (0 through two years of age) that are eligible for Early Childhood Special Education services.

Essential job functions include the following:

- Facilitate the development, implementation and evaluation of the Individual Family Service Plan (IFSP) for young children with special needs and their families.

- Assist families in identifying their concerns and priorities of these concerns for their entire family as it relates to their young child with special needs.

- Assist families in identifying appropriate resources, both informal and formal, in meeting their identified needs.

- Maintain accurate records and documentation related to service coordination.

- Comply with State and Federal laws, rules and regulations in regard to IDEA (PO 101-476 -- Individuals with Disabilities Education Act).

Other functions of the job include but are not limited to the following:

- Serve as a contact person for families to access community resources.
Facilitate transition of services to appropriate individual(s)/agency(ies), upon the child's third birthday or as necessary to achieve the IFSP outcomes.

- Enhance family's ability to be their own effective service coordinator.

- Other duties as necessary or assigned.
 

Requirements

Knowledge, Skills and Abilities Required:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. · BA degree in area of health, human services or related field or related experiences (minimum of four years) working with young children with special needs and their families.

· Good verbal and written communication skills. Effective organizational skills. Ability to work effectively as a member of a team.

· Knowledge of infant through toddler development.

· Understanding of and respect for racial, cultural, and/or ethnic differences and various coping styles.

· Ability to establish and maintain cooperative and meaningful relationships with families, representatives of agencies and other community members.

· Ability to be flexible with families and others in relation to daily/weekly schedules.

· Knowledge of community resources for individuals with special needs.

· Ability to understand, interpret, apply, and explain rules, regulations, laws, policies, and procedures.

Physical Requirements:
Regularly required to sit, stand, walk, talk, hear, operate a computer and other office equipment, reach with hands and arms, and must occasionally lift and/or move up to 10 pounds. Vision abilities required include close vision.
